Preparing for Your Visit
What to Bring With You
We ask that you bring your insurance card and photo ID with you to every visit. If you have previous medical records, please bring them as well or have them mailed to our office. Your provider will also need a list of all your current medications including the dose and the times you take them.
Your Registration Process
Please arrive to your appointment 15 minutes early to complete the registration process or you may fill out the forms prior to your visit on the patient portal. You will also be asked to sign our HIPAA Acknowledgement, as required by law.
Please be prepared to pay your deductibles, co-pays, co-insurance or any other balances at the time of your visit. We accept a variety of payment options including cash, check, or major credit cards. What to Expect
From the moment you enter our office you will be treated with care and compassion.
The Annual Well Women Exam includes:
- Comprehensive history and physical: Your provider will ask you a few questions about your sexual, medical and family history and then will perform a physical exam to check your overall health.
- Breast exam: Your provider will inspect and palpate your breasts and your underarms while your arms are in various positions.
- Pelvic exam & pap smear: Your provider will examine your reproductive organs for problems and check you for cervical cancer.
- Wellness Counseling: Issues such as diet and exercise, smoking, self breast exam, menopausal symptoms and hormones.
